Introduction
Men’s Low-Intensity Residential

The following three forms must be completed and submitted to Admissions@LifelineConnections.org or faxed to: (360) 397-8455 prior to admission.
The unique treatment program has staff onsite 24/7 to ensure everyone’s safe recovery. Residents participate in a minimum of five treatment hours each week along with individual and group therapy sessions.
