H.O.S.T. (Homeless Outreach Stabilization and Transition)
Lifeline Connections’ HOST (Homeless Outreach, Stabilization and Transition) Team engages individuals experiencing homelessness with severe and persistent behavioral health conditions.
They reduce barriers to accessing traditional treatment programs.
